As far as having general side effects, grapefruit extract seems to have none of those. It does, however, have some interactions with different medications.
If you are taking any medications, you may want to ask your doctor if grapefruit extract will interact. Don't ask their opinion on the supplement itself though. Many doctors couldn't care less about herbal supplements and they often only have negative things to say, while they promote medicines that cause harm to the body.
Grapefruit extract may especially interrupt the effectiveness of estrogen and calcium channel blockers. Grapefruit extract may decrease the absorption of certain medications in your body. That's why if you are taking a prescription, it's important to find out how it interacts with grapefruit extract.
If you are pregnant or nursing, it would be perfectly safe to eat grapefruits. However, if you want to take a grapefruit extract, there is not enough known about it for me to give you the go-ahead. You can however use grapefruit itself to help you.
